Issues with LIR Portal

Since 02:00 (UTC+2) we have been experiencing issues with our document management system, which is intermittently affecting request forms and transfer requests in the LIR Portal.

Update: The issue has now been resolved, a number of scripts were broken after a restart and this caused a problem in our document management system.
