First draft of the European Template for IP number requests

Just as a support for PR actions we could ask requestors how they managed to
contact the local IR or RIPE-NCC, i.e. where they got the address from.
Of course this question is optional and we might increase the chance of
getting answers if we ask in a multiple choice manner.
