[Accountability-tf] Draft Accountability Task Force Report - Please Review

Great work so far.

I have made some comments in the Google docs.

Mostly references to existing material.

May be worth while to have Mirjam and Daniel also read trough and provide
input.

Especially on the places where it says "does not exist"/"have not been
discussed".
