Some islands still exist (and one of the problems is "figuring out 
where things are broken again this week")

Same here.  OTOH, there are some rumors that Deutsche Telekom is working
on multicast-enabling their ADSL infrastructure, and that FIFA is 
considering multicast-streaing the upcoming football (soccer, whatever)
events...
 
And video streaming *such* events *is* expensive :-)
