With regard to subscription/posting to mailing lists:-

IMHO, subscription should (except in special cases) imply posting
access; additionally, you should be able to gain posting access for an
alternative address without having to subcribe that address to the
list.

With regard to spammers attacking mailing lists:-

Whatever scheme you invent to make it hard for attackers to subscribe
or post, someone could subscribe to the list (or jump through any
other hoops required to get posting permission) for long enough to
send a spam and then unsubscribe (or abandon the throwaway account
which they'd done this with).

...which is obvious; but the point is that with standard software such
as Majordomo it should be possible to do this automatically for many
lists, no human intervention required.

AFAICS the only way to prevent this is for every list to have a
different trick required of the user to post, and for that trick to be
expressed only in natural language.

Of course some mailing lists are still entirely manually managed.
